5 Principles for Improving End-of-Life Care

ihi_conversation_ready_imgJuly 31, 2015 — The Institute for Healthcare Improvement’s has released a white paper called “Conversation Ready:  A Framework for Improving End-of-Life Care.” Based on work that began in 2012, it includes a detailed framework built on five core principles to help healthcare organizations engage patients in conversations about their wishes for end-of-life care, steward that information, and then respect those wishes at the appropriate time.

The white paper describes the changes associated with the framework’s five principles that can be implemented to improve end-of-life care, provides examples of changes tested by organizations, and suggests measures to guide improvement.
